This document is Assessment 2 of the two (2) assessments you are required to complete, together with structured workplace learning, for CHCLAH004 Participate in the planning leisure and health programs for clients with complex needs.

Read the case study below and answer the following questions in order to demonstrate your knowledge of participating in planning leisure and health programs for clients with complex needs.
Case study 1
Tammy is 15 years old. She lives with her grandmother, Ruby, in a two-bedroom villa in a suburb approximately 40 minutes from the closest major city. Tammy was raised by Ruby after her parents decided they were unable to cope with her behavioural issues. At the age of six, Tammy was identified as having autism and minor hearing loss. She has attended a school for children with special needs since her grandmother became her guardian.
Since starting high school, Tammy has rarely attended. Her grandmother increasingly needs Tammy to help with routine tasks at home and to care for her as her health has deteriorated following a stroke.
Tammy is prescribed medication for recently diagnosed Attention Deficit Hyperactivity Disorder (ADHD) and depression; however, Ruby only gives Tammy her ADHD medication when their daily routine includes tasks she ‘needs to concentrate on’. Ruby is also worried the medication for depression is contributing to Tammy’s weight gain.
Max, a health and leisure worker, recently met the family and recorded the following notes during the interview:
• Tammy – erratic behaviour, constantly moving, doesn’t respond appropriately to questions (not sure she could hear me clearly – check when her hearing aid was most recently serviced); understands basic instructions; some literacy (has this been assessed?); seems quite overweight (Obese? When was she last assessed by her doctor?).
• Ruby says Tammy has only one friend who she sees occasionally. Rarely leaves the house except for errands (she accompanies Ruby when shopping, travelling by taxi).
• Ruby advises that Tammy loves music and dancing, enjoys television and picture books. Tammy recently attended a ‘movement class’ advertised in the local paper; however, she was unhappy that the ‘special kids’ were in a separate group. Wiggles and Hi-5 songs were used for the activities. Tammy has a large collection of dolls she enjoys dressing and she also enjoys learning to cook basic dishes under Ruby’s supervision.
• Ruby is finding it hard to stay in contact with her friends since her stroke – particularly misses her friends from lawn bowls. She has limited mobility – uses a wheelchair and walking frame. She misses knitting and embroidery, and now does Sudoku and crosswords. She would like to know how to use her new home computer to stay in touch with friends.
• Has a service attending to mow lawns and do light gardening – Ruby used to be a florist and takes great pride in her roses.
• Doctor told Tammy needs to watch her weight – she isn’t exercising enough and her diet includes too much pasta and rice. Ruby is concerned about what will happen to Tammy if Ruby eventually has to live in an aged care facility.

Case study 2
Read the case study below and answer the following questions in order to demonstrate your knowledge in relation to participating in planning leisure and health programs for clients with complex needs.
After being released from gaol several months ago, Brandon decides not to return to his remote community and instead relocates to a major city.
Brandon’s social worker, Ben, considers the impact of Brandon’s Aboriginal background, his age (40 years), health and employment history and agrees that this move offers Brandon a fresh start with improved employment opportunities and new peer group.
Brandon is determined not to return to his history of alcohol and drug misuse as he realises that this led to the violent behaviour he was convicted for. During this time in gaol, he took advantage of the literacy, language and numeracy programs on offer; however, he has limited work experience and technical skills.
Brandon also has additional health challenges. He was recently diagnosed with diabetes and has some degree of dementia as a result of his positive HIV status.
