Should your 1st attempt be not satisfactory, your teacher will discuss the relevant questions with you and will arrange a 2nd attempt to be scheduled. Should your 2nd attempt not be successful, or you fail to undertake the 2nd attempt, you will be deemed “not satisfactory” for this assessment item.
Only one re-assessment attempt may be granted for each assessment item.

Nursing Care Plan
Scenario
Philip Lusk is a 69 year old Italian man who lives with his wife in a low-care facility. He has recently been diagnosed with early stage Dementia. Mr Lusk has behavioural and psychological symptoms of dementia. Every day he paces around the facility in an agitated way, and raises his voice to fellow residents. Philip has little to no understanding of what is said to him and he has minimal language to communicate his needs. Philip has been prescribed a PRN dose of Risperidone 0.5mg for agitation.
He responds to people’s body language and non-verbal cues. If he is unsure of what is happening around him or he feels threatened by people body language/tone of voice, he may react in an aggressive manner, or may become repetitive in his speech.

Describe in detail how you would manage this behaviour through psychosocial interventions. Your answer should include headings verbal and non-verbal communication and environmental changes. Your care plan should Identify areas that you would consider in nursing care.
Including :
1. Establish a therapeutic environment within the scope of Enrolled/Division 2 nurse practice
2. Demonstrate communication styles to de-escalate Behavioural and Psychological Symptoms of Dementia (BPSD)
3.
Under the heading “Environment”, “verbal communication,” and “non-verbal communication” outline nursing intervention to de-escalate agitation, ensuring that you address how you would apply ethical and moral concepts to all clinical interventions
4. Ensure work practices reflect understanding of the interrelationships of the needs and rights of the individual, the family, the community and society
5. Your care plan should have the date of development and a review date.
6. Under heading “person responsible”, state who could perform these actions for example nursing staff, family etc.
